The Kidney Foundation of Canada stands firm for every Canadian living with or at risk of kidney disease – no matter who they are, where they live, or where they are on their kidney journey.

Meeting that commitment takes more than vision. It requires clarity, focus, and a shared plan of action. Our new Theory of Change is that plan: a roadmap that outlines the impact we’re working towards and how we’ll achieve it.

Developed through nationwide consultation, it reflects the realities of kidney disease in Canada today and the urgent need for action.

Over the next five years, we will change the course of kidney disease by:

  • Empowering people to take control of their kidney health and easing the burden of kidney disease through connection and support;
  • Breaking down the systemic barriers that delay, complicate, or limit equitable access to treatment and care;
  • Investing in research that leads to better prevention, earlier detection, and innovations in transplant and treatment.

We invite you to explore our Theory of Change and to walk with us as we work to create lasting, meaningful change for all those affected by kidney disease.
